By accessing this website, scheduling an appointment or purchasing a service or product, you acknowledge that you have read, understood and agreed to these terms.

ABOUT ROOT REVIVAL

Root Revival Hair Restoration provides trichology evaluations, hair and scalp imaging, non-surgical hair-restoration services, scalp-support services, advanced testing, alternative hair solutions and educational guidance.

Root Revival Hair Restoration is owned and operated by Suzi Watkins, Board-Certified Trichologist.

RESULTS ARE NOT GUARANTEED

Hair growth and scalp response vary between individuals. Results may be influenced by genetics, medical conditions, medications, hormones, nutrition, stress, lifestyle, consistency, treatment timing and other factors outside the control of Root Revival Hair Restoration.

No specific outcome, amount of growth, change in density, reduction in shedding or improvement in scalp condition can be guaranteed.

Before-and-after photographs, reviews and client testimonials represent individual experiences and do not guarantee that another client will receive the same results.

APPOINTMENTS AND ELIGIBILITY

Clients are responsible for providing complete and accurate information about their health history, medications, allergies, current treatments and relevant medical conditions.

Root Revival Hair Restoration may decline, postpone or discontinue a service when:

Required intake forms have not been completed
Relevant health or medication information has not been provided
A possible medical condition requires evaluation or clearance
A service is not considered appropriate for the client
A safety concern or contraindication is identified
A client’s conduct creates an unsafe or inappropriate environment

Certain treatments require a Comprehensive Hair & Scalp Evaluation before treatment can be performed.

Clients under 18 must have permission from a parent or legal guardian. A parent or legal guardian may also be required to attend the appointment and complete consent documents.

SCHEDULING AND COMMUNICATION

Appointments are available by advance scheduling.

Root Revival Hair Restoration communicates primarily through text and email. This allows Suzi to remain fully present during appointments and provides a documented record of scheduling information, recommendations and next steps.

In-depth questions about hair loss, treatment suitability or scalp concerns may require a scheduled Hair & Scalp Evaluation and cannot always be appropriately addressed through text or email.

Submitting a request does not guarantee an appointment. An appointment is confirmed only after you receive confirmation from Root Revival Hair Restoration or its authorized scheduling platform.

CANCELLATIONS AND RESCHEDULING

Please cancel or reschedule at least 24 hours before the scheduled appointment.

Appointments cancelled or rescheduled with less than 24 hours’ notice may be subject to a $75 rescheduling fee. An invoice may be emailed, and payment may be required before another appointment is scheduled.

Root Revival understands that illness and emergencies occur and may allow one late rescheduling accommodation. Repeated late cancellations or rescheduling may be treated as cancellations and subject to the applicable fee.

Failure to attend an appointment without notice may result in the loss of any deposit or appointment payment and may require payment before future appointments are accepted.

PAYMENTS, PACKAGES AND REFUNDS

Payment is due at the time specified when scheduling, purchasing or receiving a service.

Accepted payment methods may include major credit cards, checks, cash and approved third-party financing.

Because services, evaluations, reports, testing and treatment plans are personalized, all services and packages are non-refundable once purchased or initiated unless otherwise stated in writing.

Unused services may be transferred only with prior approval from Root Revival Hair Restoration. Transfers are not guaranteed.

Once a customized report, test, ordered product, hairpiece or personalized service has been initiated, processed or ordered, the associated payment may not be refundable.

Any additional purchase, shipping, financing or return terms displayed at checkout or included in a written agreement become part of these Terms of Service.

Georgia does not require businesses to provide refunds in every circumstance, but businesses must honor the refund policies they communicate to consumers. Root Revival will honor the applicable policy disclosed at the time of purchase.

ADVANCED TESTING

DNA, epigenetic and other advanced testing services provide information that may be used to support education and personalized planning.

Testing does not independently diagnose a medical condition and does not replace medical testing, bloodwork, medical examination or care from a licensed healthcare provider.

Results are dependent upon the information, methodology and reporting provided by the applicable third-party laboratory or testing company. Root Revival Hair Restoration is not responsible for laboratory delays, shipping delays, sample-processing issues or changes in third-party testing procedures.

Recommendations involving prescription medications require review and authorization by an appropriately licensed medical provider. Root Revival Hair Restoration does not prescribe medication.

THIRD-PARTY SERVICES

The website may contain links to or integrations with third-party services, including scheduling platforms, payment processors, financing providers, pharmacies, laboratories, medical providers and product manufacturers.

Root Revival Hair Restoration does not control and is not responsible for the availability, privacy practices, content, policies, decisions or performance of independent third-party services.
